Job Overview
As a Home Health Registered Nurse, you will provide skilled, compassionate nursing care to patients in their homes while collaborating closely with physicians and an interdisciplinary care team. This role includes comprehensive patient assessments, development and management of individualized care plans, medication monitoring, and delivery of physician-ordered skilled nursing services. You will educate patients and caregivers, coordinate services with aides and clinicians, supervise home health aides, and maintain accurate clinical documentation. The ideal candidate is clinically strong, organized, patient-focused, and comfortable working independently while making a meaningful impact on patient safety, independence, and quality of life.
