Join our Springfield, MA team! We’re hiring a Registered Nurse, Transition of Care, offering $50–$60 per hour to support older adults through safe, well-coordinated care transitions.

Requirements to Apply:
• Active Registered Nurse (RN) license in the state of Massachusetts
• Graduate of a board-approved nursing program
• At least one year of RN experience with frail or elderly populations in community-based, long-term care, or similar settings
• Ability to work independently while collaborating effectively with an interdisciplinary team

Registered Nurse, Transition of Care full-time role with a primarily daytime schedule, supporting frail and elderly adults through a PACE-based, community-focused care model.

Job Overview:
The Transition of Care RN plays a critical role in ensuring seamless, high-quality transitions for participants moving between hospitals, skilled nursing facilities, home health, and the community. Working closely with the interdisciplinary team, this nurse serves as the primary liaison among providers, facilities, participants, and families to ensure continuity of care. Responsibilities include coordinating care during hospitalizations and discharges, supporting safe discharge planning, conducting medication reconciliation, providing participant and caregiver education, and maintaining accurate EMR documentation. The ideal candidate is organized, clinically strong, and passionate about serving older adults, with the ability to build trusted relationships and advocate for person-centered care that helps participants remain safely in their community.
