Job Summary
We are seeking a Physical Therapist to provide care across outpatient, inpatient, swing bed, and long-term care settings. Approximately 70–75% of the caseload will be outpatient, with additional coverage supporting other rehabilitation services as needed.

Responsibilities
Evaluate patients and develop individualized treatment plans
Provide physical therapy services across outpatient, inpatient, swing bed, and long-term care settings
Monitor patient progress and modify treatment plans as appropriate
Educate patients and caregivers on exercises, mobility, and injury prevention
Collaborate with physicians, nurses, and interdisciplinary care teams
Maintain accurate and timely clinical documentation
Participate in discharge planning and care coordination

Requirements
Graduate of an accredited Physical Therapy program
Active Physical Therapist license or eligible for licensure
Strong clinical assessment and treatment planning skills
Ability to work independently and as part of a multidisciplinary team
