Overview
Join a mission-driven team supporting older adults through the PACE (Program of All-Inclusive Care for the Elderly) model. As an Occupational Therapist, you'll play a vital role in helping seniors maintain independence and quality of life. This is a Monday–Friday, daytime position with mileage reimbursement and a collaborative, interdisciplinary care setting.
Key Responsibilities
Assess physical, cognitive, and functional needs of geriatric participants in clinic, home, or facility settings
Develop and implement individualized therapy plans focused on maintaining or improving daily living skills
Provide education on adaptive equipment, mobility aids, and techniques to participants and caregivers
Collaborate closely with an interdisciplinary team to ensure coordinated, whole-person care
Required Qualifications
Licensed Occupational Therapist in California
Minimum 1 year of experience working with older adults or in geriatric rehabilitation
Valid California driver’s license, reliable transportation, and ability to travel to participant locations
Strong clinical judgment, communication skills, and team-based approach to care
Preferred Qualifications
Bilingual English/Spanish
Experience with PACE or community-based care models
Familiarity with CMS and DHCS regulatory requirements
Skilled in documentation and electronic health record systems
