PACE Home Care Coordinator Job in Chula Vista, CA – Fulfilling Community Role – Full-Time with Field & Office Balance

Job Overview
We are seeking a compassionate and detail-oriented PACE Home Care Coordinator to support our interdisciplinary team at the Chula Vista location. In this hybrid field/office role, you will coordinate unskilled home-based care services—including meal delivery, caregiver visits, and durable medical equipment—for seniors enrolled in the PACE program. You’ll advocate for participant needs, conduct home safety evaluations, and ensure services are delivered in alignment with participants' goals and care plans. Most days include 1–3 home visits (30–60 minutes each), with the remaining time in-office.
