Join our Southwick, MA team! We’re hiring a Group Adult Foster Care Manager to support older adults and individuals with disabilities in the community, earning $25 per hour.

Requirements to Apply

  • Bachelor’s degree in social services or a related field
  • At least 2 years of recent experience working with elderly or disabled individuals
  • Bilingual in English and Spanish
  • Strong communication, organizational, and basic computer skills with the ability to work independently

Group Adult Foster Care Manager full-time role with a community-based schedule, supporting elderly and disabled adults through a mission-driven GAFC program.

Job Overview
In this role, you’ll blend social work and care management to support members enrolled in a Group Adult Foster Care program. You’ll conduct in-home assessments, complete psychosocial evaluations, and collaborate closely with a registered nurse to develop and adjust individualized care plans. The position includes regular home visits, caregiver training and supervision, coordination of community and social services, and ongoing communication with members, families, caregivers, and the interdisciplinary team. The ideal candidate is compassionate, organized, self-directed, and comfortable building trusted relationships that help participants remain safe, supported, and independent in their homes.
