Salary Range: $104,019 – $139,792 annually
Requirements to Apply
- Completion of an accredited Physician Assistant or Nurse Practitioner program
- Current North Carolina PA or NP license
- Valid DEA Controlled Substance Registration
- Current BLS certification
- Experience managing geriatric or high-risk patient populations
- Ability to practice within scope and collaborate with supervising physician
Preferred
- 2+ years of geriatric primary care experience
- Experience in PACE or value-based care models

Job Overview
This full-time Advanced Practice Provider opportunity offers the ability to deliver comprehensive ambulatory primary care services to a geriatric population in a structured outpatient setting. Providers manage scheduled patient visits, provide clinical coverage during center operating hours, and respond to telephone consultations.
Responsibilities include assessment, diagnosis, and management of acute and chronic conditions, documentation and administrative tasks related to patient care, and participation in quality improvement initiatives. Providers are expected to manage a broad range of medical conditions within their privileges and consult with supervising physicians for complex or severe cases.
This role is ideal for an experienced clinician passionate about caring for seniors and improving outcomes in a coordinated, team-based care environment.
Schedule: Monday – Friday, 8:00 AM – 5:00 PM (Full-Time)
