Pay Range: $42–$55/hr
Requirements to Apply
- Bachelor’s degree in Physical Therapy from a CMS-approved institution
- Active California Physical Therapy license
- Minimum 1 year experience working with frail or elderly populations
- Valid California driver’s license, personal vehicle, and auto insurance
- CPR and First Aid certification (within 6 months of hire)

Job Overview
We are seeking a Physical Therapist to provide participant-centered therapy services within a local PACE program. This role supports older adults through assessment, rehabilitation, and coordinated interdisciplinary care, with services delivered across clinic settings and directly in participants’ homes.
The ideal candidate is flexible, collaborative, and committed to improving mobility, safety, independence, and quality of life through evidence-based physical therapy.
What You Will Do
- Conduct initial assessments and ongoing reassessments
- Develop and implement individualized Plans of Care with the Interdisciplinary Team
- Deliver therapeutic interventions and monitor participant progress
- Assess needs for equipment, orthotics, and prosthetics
- Educate participants and caregivers on exercises and safety
- Coordinate care with external therapists during hospitalizations
- Complete timely, accurate documentation in the EMR
- Participate in quality improvement initiatives and program development
- Maintain compliance with clinical, professional, and regulatory standards
- Support safety practices, emergency preparedness, and proper body mechanics
