Job Overview
As a Home Health Physical Therapist, you will deliver personalized, hands-on therapy to patients in their homes, helping them restore function, reduce pain, and maintain independence. You’ll evaluate physical abilities, create individualized plans of care, and provide therapeutic interventions focused on mobility, strength, and safety. This role includes educating patients and families, monitoring progress, documenting care, and collaborating closely with physicians and an interdisciplinary care team.
