Job Overview
As a Home Health Physical Therapist, you’ll provide high-quality, hands-on therapy to patients in their homes, helping reduce pain, restore function, and support safe independence. Your work will include evaluating physical abilities, developing individualized plans of care, delivering therapeutic interventions, and educating patients and families on exercises, safety, and assistive devices. You’ll monitor progress, document care, collaborate with physicians and an interdisciplinary team, and participate in care planning and discharge planning.
