Job Overview
As a Social Worker (MSW) in the PACE program, you will be a key member of an interdisciplinary care team dedicated to helping older adults live safely and independently. You’ll complete psychosocial assessments, contribute to individualized care plans, and provide counseling, crisis intervention, and emotional support to participants and their families. This role includes care coordination, advocacy, discharge planning, and serving as a liaison with caregivers and community resources.
