Salary Range: $58,000–$85,000 + Sign-On Bonus
Requirements to Apply
- Master’s degree in Social Work from an accredited program
- Active Washington State LCSW
- Minimum 2 years of behavioral health or social work experience
- Experience with older adults, medically complex, or vulnerable populations preferred
- Valid WA Driver’s License and proof of insurance
- CPR/First Aid certification (or ability to obtain within 30 days)
- ACSW or NASW-recognized certification preferred

Job Overview
The Behavioral Health Specialist II (LCSW) provides comprehensive social work and behavioral health services to older adults enrolled in a healthy aging and wellness program. This role supports participants with complex medical, behavioral health, and social needs through assessment, counseling, advocacy, and care coordination.
As a core member of an interdisciplinary care team, the LCSW collaborates closely with providers, caregivers, and community partners to promote independence, safety, and quality of life. Services are delivered across clinic, home, and community settings, with a strong emphasis on aging in place and whole-person care.
What You Will Do
- Conduct in-person psychosocial assessments and ongoing reassessments
- Provide individual therapy, counseling, and crisis intervention
- Collaborate with the care team to develop and update comprehensive care plans
- Serve as a liaison between participants, caregivers, providers, and community agencies
- Support transitions of care, hospital discharges, respite stays, and long-term placement as needed
- Conduct home visits to assess safety, support systems, and living environments
- Assist with housing stability, benefits navigation (SSI/SSDI), and Medicaid eligibility
- Support advance care planning, including MDPOA, DNR, and proxy documentation
- Maintain timely, accurate clinical documentation
- Advocate for participants to ensure continuity of care and community stability
