Home Health Manager (RN)
$109,240 – $144,197 annually + bonus

Bachelor’s degree required (additional relevant experience may be considered in lieu of degree)

Minimum 2 years of nursing experience in a home health setting, preferably with frail or elderly populations

We are hiring a Home Health Manager (RN) in Elk Grove, CA to lead and support high-quality home-based nursing care for a medically complex senior population within a value-based care model.
Job Overview
The Home Health Manager is responsible for overseeing the delivery of exceptional clinical care in participants’ homes, as well as care provided in nursing homes and board and care settings. This role leads and supports home health nursing staff, ensures compliance with care plans and workflows, and collaborates closely with the interdisciplinary team to optimize participant outcomes.
Key Responsibilities

Direct a high-performing home care department, ensuring workflows, policies, and clinical standards are consistently followed

Hire, train, coach, and support home care nursing staff to promote accountability and excellence

Evaluate the effectiveness of nursing assessments and interdisciplinary team participation

Ensure accurate delivery and documentation of nursing interventions aligned with participant care plans

Oversee and monitor quality of care provided in nursing homes and board and care facilities
