Full-time, mission-driven Physician Assistant role providing participant-centered care to a nursing home–eligible senior population in a collaborative PACE setting with clinic-based and in-home visits.

Job Overview
We’re seeking an experienced Physician Assistant to diagnose and treat a nursing home–eligible senior population who wish to remain living independently. In this role, you will function as a key member of the interdisciplinary team, helping develop care plans that support participant goals and outcomes. This position is primarily based in a PACE center setting with some in-home visits and follows a Monday through Friday schedule, ideally 8:00 a.m. to 5:00 p.m., with a 1-in-4 phone-only call rotation. Providers typically manage a caseload of 60–85 participants, with daily schedules varying between clinic visits and home visits. The ideal candidate brings strong geriatric and PACE experience, clinical independence, and a collaborative approach to caring for complex senior populations.
