PACE Primary Care APP (Nurse Practitioner or Physician Assistant)
Salary Range: $135,000 to $160,000 annually
Requirements to Apply
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Active New Jersey NP or PA license
- DEA registration and CDS certification
- BLS certification (or ability to obtain within 90 days)
- Strong clinical assessment and primary care skills
- Ability to work within an interdisciplinary, team based care model
Preferred
- Board Certification in Geriatrics
- 2 or more years of direct patient care experience
- Experience working with geriatric or frail elderly populations

Job Overview
This Primary Care APP (NP or PA) role delivers comprehensive, longitudinal care to a geriatric population within a PACE (Program of All Inclusive Care for the Elderly) model. The provider works closely with an interdisciplinary team (IDT) to assess, plan, and manage care, ensuring continuity across all care settings.
The APP is responsible for performing detailed evaluations, managing chronic and acute conditions, coordinating care plans, and supporting safe transitions of care. This role emphasizes thoughtful clinical decision making, minimizing unnecessary testing and referrals while maintaining high quality, patient centered outcomes.
