Salary Range: $105,000 to $112,000 annually + $5,000 sign on bonus
Requirements to Apply
- Graduate of an accredited Physical Therapy program
- Active California Physical Therapist license
- Valid driver’s license and proof of auto insurance
- Strong clinical assessment and treatment planning skills
- Ability to work within an interdisciplinary, team-based care model
Preferred
- Bilingual in Hmong
- 2 or more years of experience in hospital, skilled nursing, or community-based settings
- Experience working with geriatric or frail elderly populations

Job Overview
This Physical Therapist role provides comprehensive rehabilitation services to a geriatric population within a PACE (Program of All Inclusive Care for the Elderly) model. The role includes conducting in person assessments, developing treatment plans, and delivering therapy across multiple settings including the clinic, participant homes, and contracted facilities.
