Requirements to apply
• Bachelor’s degree in Physical Therapy (Master’s or Doctorate preferred)
• Active California Physical Therapist license or eligibility
• Minimum 1–2 years of clinical experience preferred
• Experience with frail or elderly populations preferred
• Ability to work in interdisciplinary, team-based care models
• CPR and First Aid certification (or ability to obtain within 180 days)
• Strong communication, collaboration, and patient care skills

Job Overview
We are seeking a compassionate Physical Therapist to deliver high-quality, patient-centered care focused on improving mobility, safety, and independence. This role emphasizes continuity of care and long-term relationships with patients, rather than high-volume, short-term treatment models.

Responsibilities include conducting physical therapy assessments in clinic and occasional home settings, developing individualized care plans, and delivering therapeutic interventions such as gait training, strengthening, functional mobility training, and modalities. The Physical Therapist collaborates closely with an interdisciplinary team to coordinate care, participates in care planning meetings, and provides education to patients, caregivers, and staff.

Additional responsibilities include supervising Therapy Assistants, supporting functional maintenance programs, ensuring accurate documentation, and maintaining quality standards in a patient-centered environment.
