Salary Range: $130,000 – $140,000 annually


Requirements to Apply

• Graduate of an accredited Nurse Practitioner program 
• Active New Jersey Nurse Practitioner license 
• CDS certification and DEA registration 
• BLS certification (within 90 days of hire) 

Preferred

• 2+ years of direct patient care experience
• Experience working with frail or elderly populations 
• Board certification in Geriatrics

Job Overview
This Nurse Practitioner role offers the opportunity to deliver comprehensive, relationship-driven primary care within a PACE (Program of All-Inclusive Care for the Elderly) setting. You’ll work closely with an interdisciplinary team (IDT) to assess, plan, and manage care for a medically complex, primarily geriatric population focusing on continuity, prevention, and whole-person care rather than episodic treatment.

In this role, you’ll manage participant care plans, oversee medications, coordinate with specialists, and support care transitions across settings. You’ll play a key role in daily IDT collaboration, contribute to quality improvement initiatives, and help guide clinical decision-making that prioritizes appropriate utilization of resources.
