Registered Nurse- RN- Case Management

Relocation and 6k sign on bonus

Responsibilities:

• Screen patients for post-acute care needs
• Develop transition plans and coordinate care
• Educate patients and families
• Make referrals to community care services
• Continuously monitor, update, and reassess care plans

• Nursing degree/diploma required upon hire.
• Bachelor's degree in Nursing or higher within 3 years of hire.
• Oregon Registered Nurse License upon hire.
• 2 years of Acute care hospital or community-based experience.
• 1 year of experience with electronic medical records or documentation systems.
• 1 year of Home health, mental health, substance use, hospice, utilization review, discharge planning, or care management experience.
• CCM preferred
