Shift: Day Shift, 8AM - 4:30PM

Job Summary:
A medical facility in Beloit, WI is looking for a full-time, permanent RN Case Manager to join their Hospice team! The Case Manager will be responsible for the provision of care for an assigned group of patients/family based on the nursing process. The Case Manager will also be responsible for the coordination of services provided by the Interdisciplinary Team assuring that the patient and the family needs are met, and case is provided consistent with federal and state regulations, policies and procedures, and hospice standard of practice. 

Job Requirements:

• Graduate of accredited nursing program
• ADN required, BSN preferred
• Unrestricted WI or IL License with the ability to get the other upon hire
• Valid Driver's License
